You will be asked to submit the following documents along with your online application, or to the Radiologic Technology Office, no later than May 28. Please submit your attachments as PDF or Word files only.
- Unofficial Transcripts
Copies of your unofficial transcripts.- Any/all previous college education.
- Proof of high school graduation or official results of high school equivalency (applicants with a recognized higher education degree are exempt).
- CNA Training Documentation
Documentation showing completion of Certified Nursing Assistant (CNA) training or proof of previous or current certification as a CNA or acceptable approved alternative. - Pre-Admission Testing: HESI Entrance Exam
A copy of your Health Education Systems, Inc. (HESI) Entrance Exam results.
